The Certificate of Insurance (Certificate), provided to you once coverage has been issued, will contain the specific information on each benefit and additional information (including the eligibility rules for participation) can be found in the Insperity Welfare Benefits Plan Summary Plan Description (SPD).

 

Note: If you apply within 30 days of your Benefits Eligibility Date, you will not have to take any medical tests or answer any health questions for certain levels of VTL and/or disability insurance, referred to as Guaranteed Issue amounts. If you apply after 30 days of your Benefits Eligibility Date or for amounts in excess of the Guaranteed Issue amounts, you will have to provide proof of good health. You can apply for AD&D insurance at any time without proof of good health.

 

Effective January 1, 2022, the Group Universal Life (GUL) insurance policy will terminate and be replaced with a Voluntary Term Life (VTL) policy. If you are currently enrolled in GUL coverage, you will receive more detailed information about the termination of the GUL policy and the new VTL policy by mail.
